Please note!
Before you start with the form, make sure you have the following items ready (in PDF format or as high-resolution jpg/jpeg or png files, each not exceeding 8mb):
Landlord's statement if you live in a rented property.
Bank statements from the past month (for your income only).
Copy of the back and front of your identification (please redact as per GDPR; otherwise, we cannot process this).
Copy of your bank card (no need for a copy of the guarantor's bank card).
Bank statements for your income and any BKR (Credit Registration Office) payments.
BKR report/print screen.
For employees:
Employer's statement not older than 1 month.
If the employer does not provide an employer's statement, for example, because the employment has not yet begun, please provide the employment contract and the full contact details of the relevant person within the company.
Payslips from the last 3 months. If the employment is less than 3 months old, please provide only the payslips issued by the employer.
For self-employed individuals:
Digitally certified extract from the Chamber of Commerce, not older than 1 month.
If available, the most recent annual statement.
If available, payslips from the last 3 months.
For students:
Confirmation of admission to your educational institution.
Confirmation of receiving financial aid (DUO).
If you are on an internship, a confirmation letter from the company.
For individuals receiving benefits:

If you have a legal guardian:
If you have a guarantor:
If you are using a guarantor, ensure that this individual completes and signs this document in full. You will need to upload this document as a PDF or image if you have a guarantor.
Lastly:
Depending on the amount of uploaded files the form can take a little while before its done submitting. Your request will not be processed if there are missing documents.
